In his frequent messages calling for higher Latino and black turnout and his condemnation of the Arizona immigration law, Obama is trying to recapture over immigration the voter approval he lost over healthcare. Arizona acted as it did because of a lack of federal enforcement of federal law barring undocumented immigrants. With a porous border, they felt that they were left no choice but to pass a law allowing potentially intrusive searches to ferret out illegal immigrants. Because this law could subject American citizens of Hispanic origin to undue scrutiny and perhaps to needless trips to the police station, the Arizona law antagonizes the jump ball in the immigration debate -- Latino-American voters. If there were a real national identification card, the requirement to produce papers might be less intrusive. But, as it is, with the burden of proof on the citizen (or the illegal immigrant), the law is bound to raise tensions between Anglo cops and Latino citizens. But being forced to support or oppose the Arizona law is a false choice. It reflects the unimaginative politics of confrontation that jeopardize race relations and elect demagogues like Barack Obama. (Obama's share of the white vote was the same as Kerry won in 2004. He was elected only because of a three-point increase in black turnout and a shift in Latino votes to his corner. He won because of race, and now he schemes to keep control of Congress by using the immigration issue.) The real answer is not to round up Latinos in the streets of Phoenix and hope to catch illegals in the net. Nor is it even to pretend that we can stop determined men and women from crossing the border by way of more guards, more troops and better equipment. The answer is to dry up the will to cross the border in the first place by stopping employers from offering jobs to undocumented workers. If there were felony penalties -- jail time -- for hiring illegals, they would not be hired. And if there were no jobs, there would be no illegal immigration. The Republican position on illegal immigration should be to demand tough employer sanctions, including jail, and coupling that program with a vigorous guest-worker program to bring needed workers in legally, pay them a living wage and then escort them out when they are no longer needed. The United States, in need of a younger population to pay for our current and future retirees, should also raise the allowed levels of immigration. Both parties are hypocritical on immigration. Democrats, controlled by unions, want Latinos to vote but not work. Republicans, controlled by agribusiness interests, want them to work but not vote. The answer is to stand up to union and to agribusiness pressure and take tough action to stop the hiring of illegal immigrants. If there were no jobs for illegals outside of guest-worker programs, there would be no need for amnesty. They would all go home of their own accord or wait until they got legal status. We would need a foolproof, biometric identity card to speed identification of those eligible for employment to accompany the sanctions against hiring illegals, but this is a small price to pay for an answer to so pressing a problem. But Obama will not take the step that could end illegal immigration. Why? Because he wants immigration. He seeks to reshape the partisan balance in America by increasing the number of Latino voters and marrying them to the Democratic Party by provoking Republicans who just want law and order to appear racist to Hispanic-Americans. His is a game of great duplicity and racial opportunism. But good legislation can defeat his designs and solve one of our most pressing domestic problems at the same time. I agree with this: Take away their reasons for being here and most will go home. 1. $10,000 fine for each illegal a business has working for them 2. Make illegals ineligble for any benefits except emergency medical care and only to staplize them before deporting them 3. Change the Anchor Baby Law to where at least one parent has to be a legal resident of the United States 4. Disclaimer: First, I'm a physician from a third world country so I may not be 100% familiar with some of the problems/controversies that the US healthcare system has, but this is based on what I understand from the news reports On one side of the debate are the people who cant afford private insurance. * No treatment, we die. Simple On the other side are the people who are against universal health insurance. Their reasons are more complex like: * who's going to pay for it? more taxes probably * this will drive private insurance out of business * when government health insurance is a monopoly because private insurers are out of business, then it will not be inclined to improve itself because of lack of competition. what if it decides to enact harsh requirements to cut costs? If its a monopoly you have no alternative. Its too late I propose a hybrid healthcare system based on my own country's healthcare system. * There is some sort of universal or semi universal health insurance but private insurance still exists as a sort of upgrade or total replacement. If you can afford it and you want it you can purchase an "upgrade" * This universal health insurance still has a small annual fee * Visits to the doctor will be partially covered so you have to pay a small fee to the doctor when you visit the clinic. For patients that have to be confined in the hospital, the coverage is also partial so whats left is a small fee to be paid by the patient. This small fee exists so that the human psychological reaction of "its free, lets abuse it" wont happen. * how partial is partial coverage? Make it socialized. By default say only 50%. But you can ask to be re-classified, all dependent on your income. A social worker will investigate you and the amount you eventually pay will be dependent on your income. A rich guy pays a large percentage of his hospital bill but a homeless person only pays a few dollars. * There are 3 kinds of hospitals: "pay" and "charity" to use the terminology of my country. The 3rd category is "pay" hospitals that have a separate wing or building that is run as a "charity" hospital * Pay hospitals only accept people who either pay in cash or who have the "upgraded" private health insurance. As to their organization, infrastructure and the equipment, it would resemble a current run of the mill US hospital. * Charity hospitals pimarily cater to poorer people who do not have private health insurance and only rely on the government universal health insurance but they in theory will accept anyone even those who have private insurance and those who pay in cash because they are rich. * To reduce costs, some services in charity hospitals would be removed (like in no frills budget airlines). - In my country laparoscopic surgery is so expensive compared to the savings regained in productivity and shorter hospital stay so old fashioned open surgery might be standard but you would need to compute the cost/benefit ratio for your own situation. - Frills like airconditioning, TV and private rooms would go. Charity hospitals in my country have large wards with > 50 beds per ward, 2-3 nurses per ward, open to recieve fresh air and mosquito netting and bug zappers for mosquito control. Fresh air and electric fans would cool the patients in summer and if you have snow, then the windows would close and since the building is insulated, minimal heating is required. The ward would look like a US hospital in the 1920's or ww2. - Being ugly is not a health hazard so aesthetic surgery would not be covered. Reconstructive surgery would be covered of course. - Surgeons would use cheap nylon or silk sutures to close the skin. The trade off for lower price is a worse looking scar. But if you want a better looking scar then go to the pay hospital. - No more meals in bed so unless you are too weak to move, you eat in a self service cafeteria or ask a relative to fetch you the meal - since private doctors are expensive, residents (doctors in training) and nurses in training (they both have lower salaries) would primarily be used. Of course, each resident/nurse and each patient eventually has an experienced private doctor/nurse that oversees all the medical treatment. * the ICU should be more or less the same in pay and charity hospitals except maybe for a worse nurse/doctor to patient ratio With all the frills gone, there is a powerful incentive for people to work hard so they can buy private insurance. But no one dies because coverage can be universal. Resolved Question: Does the US Military Have a Problem Retooling and Reskilling Itself?

It seems that we are always ready to fight two or three wars back. We are never ready to fight in a way that makes sense today. Really it comes down to inertia, laziness, sloth, and lack of initiative. We appear to be 100% prepared to do some more Counter-Insurgency warfare, like we did in Vietnam, and lost, and like we did in Iraq, and that situation is still unresolved. But over-the-horizon warfare (aka behind the wire strategy) is just too hard for the Pentagon to get their minds around. They would rather just use their old manuals, from the 1970's or the 1990's. It's too hard to get the National Reconnaisance Office ramped up to support over the horizen warfare, and then blend in data from NSA and CIA, and feed it all into Battle Mountain. It's too hard to complete work on Dominator, which involve 16 Predators sharing data in real time on a neural network, and having overhead viewing of 400 miles by 400 miles. New contracts would have to be written. Officers with old skills would have to be let go. New officers, maybe trained by General Hayden would have to be brought in. It's very disruptive. Washington likes same old same old -- Let's keep the old equipment, the old techniques, the old people in place. Nobody's rice bowl gets broken, nobody's turf get infringed. We love the cashflow patterns just as they are. So let's just fight 1970's style warfare, which failed then, and is highly likely to fail now, but who cares as long as the cashflow channels are not disturbed in any way. Keep those old contracts in place, maybe add some Ospreys. Is it possible that US military doctrine cannot become more innovative and more responsive to the real time situation until 500 Generals are fired and retired and off in the nursing home? Better yet, do you know the reason why? Well it's pretty sneaky, really! Device manufactures make such thinks as EKG's, CAT Scanners, Lab Test Equipment, every surgical instrument ever made, x-ray, etc. And guess who pays for these devices? The patient. Each patient is charged a use fee based on all the devices in the room with the exception of major devices called "capital equipment." Those are billed in the service, such as MRI, CAT scanner, lab test, etc. And who are the patients? 80% are the already insured. IOW, 80% of the patients are going to pay for the taxes on the Device Manufacturers, which will help cover the uninsured that will be covered on the government plan. So you might ask yourself, "Why don't they just tax us instead?" Answer: "You would then know." They don't want you to know. This way, it is almost hidden and they can say they are not taxing you, but taxing those great big rich medical companies. The problem here, unlike the Pharmaceutical companies, most of the best Device Manufacturers are based right here in the USA. No we don't import devices from China or Japan yet, as we are the best at making these things... next best is the UK. Not China, not Japan, not India... only the USA. Believe me, that pacemaker inside your chest was most likely made by Medtronic (Minneapolis, MN), the world leader... ditto that stent that allowed you to avoid open heart surgery... the same products 120 other countries use. So what will our companies do that are taxed so much? Clearly they will sell and concentrate their efforts at locations that will make them the most money... IOW, outside the USA. And with each device, it is governed just like a drug so it goes through FDA testing. These are called "Trials." Many of our schools, doctors, and teaching hospitals are given grants by the device manufacturers to do these trials. I know that there are several bills out there on healthcare, all of which have good points and bad points (from what I hear). I'm more of a foreign policy buff, but I figured I would start inching my way into the debate here. I downloaded and have been reading HR 3200, which is the bill that Fox News is out to kill (for whatever reason). So when I started reading it, I thought I would find some offensive stuff (I'm more of a financial conservative), but I have found nothing yet (it's like 1,000 pages, so I haven't read the whole thing yet) that has offended me or made me think for one minute that I wouldn't like the reforms brought about by this bill. Some key points I found in the bill itself (these are my notes): * Protects rights to keep currently held health insurance policies * Cannot discriminate based on pre-existing conditions * Limits the allowed methods of varying premiums * Prohibits discrimination in benefits * Requires parity in mental health and substance abuse disorder benefits * Ensures adequacy of provider networks * Mandates minimum coverage that includes hospitalization, outpatient hospital and clinic services (including ER visits), private practitioner services (regular doctor visits), supplies necessary for treatment, prescription drugs, rehab, mental health and substance abuse disorder services, preventive services, maternity care, well baby and well child care (oral, health, vision, and hearing services, equipment, and supplies for children under the age of 21) * Requires fair marketing practices by health insurers (uniform marketing standards) * Requires fair grievance and appeals mechanisms * Requires information transparency and accurate, timely plan disclosure using plain language * Mandates timely payment of claims by insurance companies * Creates a Health Insurance Exchange with Public Health Insurance option * Prohibits discrimination in healthcare not offered through exchange * Offers protections to whistleblowers * In order to ensure value and lower premiums, all group and individual coverage must meet a specified medical loss ratio * Ends health insurance rescission abuse, which prevents companies from abusing protections created to prevent fraud (companies unreasonably end coverage based on these protections) * Creates the opportunity for independent, external, third-party review in cases of rescission * Creates reinsurance program for retirees * Mandates affordability premium credits for individuals * “No federal payment for undocumented aliens.” * Notes that it is an individual’s responsibility to obtain healthcare * Outlines employer responsibilities should an employee choose to offer health insurance to its employees These are just a few of the more important points I have found thus far. From what I've seen, any NEW bill would be better than the system we have now (though I don't think I would support universal healthcare and drastic changes to the health delivery system). I'm going to put out a couple of my ideas and you put out yours and see what we all think. 1. Technological advances. We can deliver babies alive earlier and earlier. We can get previously infertile women pregnant. We have new scans, blood tests and a mapped genome which can look at any possible risk for just about every possible disease. 2. Obesity. 2/3 of Americans are considered overweight or obese. This puts an enormous strain on the health system. 3. Disease diagnosis. Just take a look at autism, for example, how many more kids are dx with it and then treated. ADHD, depression, cancer. We're dx far more liberally and earlier than ever before. 4. Healthcare waste. Red tape, hoops, referrals, rejections, CYA tests, overcharging for equipment, having to try a variety of drugs before finding the ones that work, etc. 5. Lobbyists. 6. Illegal immigrants using our facilities. 7. Patient demand for care. We have to have Viagra. We have to have Mirena. We are demanding pills for things which were either taken care of the hard way (change in diet and exercise), or we just suffered with because they were annoying but not necessarily life threatening. 8. Drug advances. We can grow longer thicker lashes thanks to a new medication. We can prevent dry eyes. We can not absorb fat we choose to consume. And on the less frivolous side we can lower cholesterol, ease pain, control our depressive thoughts and anxiety. What are some of your ideas? more

According to a recent ABC news story, one of the fall-outs from the recession is that some new mothers are feeling forced to take shorter maternity leaves due to concerns about finances and job stability. Just last week I interviewed for a medical assistant position, and was allowed to shadow some of their current employees. While there I witnessed completely unsanitary healthcare facilities. This facility provides diagnostic testing to individuals desiring to attain federal healthcare and assistance. The population served was typically individuals that had no permanent residence (homeless) and other minorities. Testing occurred in rooms that were maybe 5ft. by 7ft. and was full of papers and charts scattered around. All testing equipment was not properly washed! When I asked why they did not use disposable mouthpieces (for pulmonary function tests) they got extremely offensive. Immunization and syringes were not properly handled (not in sterile environments), and were mass-filled and then "labelled" with post-it notes. Some say Cell-phones must not be allowed in certain places, others believe we should be able to use our cells everywhere Many people believe that cell-phones should just be overall banned. While others (mostly teenagers) are absolutely addicted to them and see no harm. My personal opinion though is that cell-phones should just be used in emergencies and important business people only. The rest is unnecessary, and in some places should be banned for the safety of yourself and others. Using your cell while driving has already been banned and is now illegal. Personally I could not agree more. Many accidents happen because drivers are talking to their cells and aren’t paying enough attention. Research has shown that your brain cannot do everything at once, talking and listening while obeying traffic lights and avoiding accidents. John Hopkins says “ You need to put down the phone and focus because your brain can’t do everything at once”. Ever since the “no talking while driving” ban, many have bought headsets. True, that it is hand free and is allowed, but part of your mind will be on the customer you are talking to. The best advice is, ignore any calls you get while driving. Cell-phones should also be banned in classrooms. Over 560 schools across Toronto have banned students using cell phones anywhere on school property. This is because cell phones are gadgets to cheating, annoyance in class, and disruption. Many teachers find that students are sharing answers through text during a test, or just simply texting each other in class. Personally I think banning cell-phones on school property is unnecessary. If they say no to cell on school property, then this should go out to teachers and the principal too. I think that cell-phones should be allowed in school, incase of a emergency but turned off. Those who get caught using them, should just receive concequences. Another important place where you shouldn’t use your cell phone is in hospitals. This is because studies have shown that the cell-phones could interfere with hospital equiptments. Cell phones also interfere with PaceMakers as well. They don’t always affect these things, but still doctors don’t want to take the chance. Gregory will be best remembered for his pioneering work against substance abuse in deprived areas of our capital. Between the war on terrorism and the battle against drugs, the rhetoric of war is firmly rooted in the lexicon of the 21st century. And it has crept into healthcare too: we read of the “collateral damage” of chemotherapy to describe the side effects of cancer treatments. How did we get from this to: “A new strategy in the war on renal cell cancer: hitting multiple targets with limited collateral damage” – the title of a 2008 research paper. Perhaps the author succumbed to the temptation to use military jargon to spruce up his manuscript. Clearly military life cannot be distanced from the rest of the world, but do we really need terms like “killer cells” and “vaccine shots” when talking to patients? I worry about the effect of such language on patients. We ask them to “beat” illness and to “keep up the good fight”. Doctors talk of building up the person’s “resistance” and of prescribing “magic bullets” to kill off cancer cells, all of which may suit those with determined personalities. But what of the meek, the mild or those who are depressed? They may be neither inclined nor able to respond to exhortations to fight back. And unable to respond, it is possible that certain people will feel they have “failed” themselves, their families and their medical team. What about the notion that engaging a fighting spirit will protect you against a cancer recurrence? A large UK study suggests this is a fallacy: a long-term follow-up of cancer patients at the Royal Marsden Hospital found that a high fighting spirit confers no survival advantage on those who displayed extreme fortitude. There are alternative approaches when we face serious illness. One of the best is to view the process as a journey. There will be bumps along the road; some parts will be downhill while others will take more effort. The journey will offer alternative routes and it may be appropriate at times to rest a while. Another advantage of the journey analogy is that you can take a travelling companion with you. Tony Gregory died at St Francis Hospice on Dublin’s north side. I suspect he was facilitated on his final journey by the wide range of health professionals who work there. After all, the word hospice is derived from the Latin, hospes, and it originally referred to the tradition of giving shelter to those on a journey. George Orwell wrote about the powerful role of words. “Never use a foreign phrase, a scientific word or a jargon word if you can think of an everyday English equivalent,” he said. I think that a lot of the problem with the healthcare system in America is that the costs are out of control. The management has been hijacked by greedy corporate types with MBA's. The healthcare industry has been hiring when other industries have been suffering and laying off. Most people who used to work at McDonald's go take a class and work at a hospital or nursing home. Hospitals have so much expensive equipment, some that cost MILLIONS. Yet, if you don't have decent health insurance or any health insurance, most patients get charged exorbitant prices for their care! Why should anyone have to go into serious debt for care? It seems like the executives of these hospitals are taking advantage of the situation and extorting people! This is why they can hire all sorts of people, even the dregs of society, and buy all kinds of machines! What's worse, the pharmaceutical industry hikes up their costs by using their drug rep minions to buy expensive lunches, dinners and outings for doctors to bribe them! That is outrageous! I used to work at a restaurant and whenever I needed to charge a rep extra, they didn't care, they'd tell me to put on a huge tip too! All that BS about needing to keep patents in order to recoup the costs of researching the drug are total garbage! I think hospitals and drug companies need to get their costs in line! McCain only voted to support legislation for the Iraq and Afghanistan Veterans of America 58% of the time and received a "D" rating; Obama supported them 89% of the time and received a "B+" rating. 3. In April 2003 McCain urged Senate members to table a vote that would appropriate more than $1 billion for National Guard and Reserve Equipment in Iraq. 4. In October of 2003 once again McCain voted to table a bill that called for an additional $322 million for safety equipment for United States forces in Iraq. 5. Since 2004 McCain has voted twice against closing corporate tax loopholes to help fund healthcare for veterans. (I find this vote to be especially interesting.) 6. In March 2004 Senator McCain voted against an amendment that would have created a $1.8 billion reserve fund to increase Veteran's medical care. 7. Again in March 2006 McCain voted against an amendment to close corporate tax loopholes in order to increase veterans' medical services funding by $1.5 billion. 8. Henry Marsh, a senior consultant at St George’s hospital in Tooting, south London, has used the Bosch 9.6 volt battery-operated hand tool to open up the skulls of his patients to remove life-threatening tumours. Occasionally the battery has gone flat halfway through. The operation is performed with the patients fully awake – a technique that fell out of use in Britain 50 years ago. Marsh said that Ukrainians could withstand such a practice because they were “very tough”. The 58-year-old consultant travels to Ukraine twice a year to perform free operations at a clinic run by a fellow surgeon, Igor Petrovich. The handyman drill was used because the local doctors could not afford state-of-the-art equipment. When working for the National Health Service, Marsh uses a £30,000 compressed-air medical drill, but he said that the Bosch was an effective stand-in. “There’s not a huge difference,” he said. “The drill is Igor’s solution. I work as a nurse in a Rehab Center, which has a lot of patients w/ MRSA and C Diff. Does anyone know a healthcare worker who has contracted one of these diseases. I am scared of contracting one of these diseases because they are so widespread and we have to work at such a break neck pace that often proper safety equipment is not used. (I always wear gloves, but sometimes forget the other equipment)Hea, how do you take out the infection? I've never heard of this. My son will be playing football this season and he needs insurance, we dont have insurance so we are goign to buy it from the school. I took my elderly mom to the ER last night because two of her doctors were afraid that she had a blood clot and they needed the ER to use their equipment to check it out. She doesn't have a clot, thank God, but oh-my-gosh, what a nightmare being in the ER waiting room for 4 1/2 hours watching all the really sick or injured people, and then seeing other people come in with a mild rash or splinter and yelling at the receptionist because they had to wait so long. The trouble-makers brought their friends, neighbors, neighbors kids, buckets of chicken and bags of other fast food, laughed and partied like it was all a big joke. Why is healthcare so expensive in this country? The people who use medical cards go to the ER for non-emergencies, and then the dr gets paid about 1/10th of what the bill actually is, leaving the rest of us to make up the difference.Mypassion, where do you think the money comes from for the medical cards???
